Do your upper eyelids droop over your eyes? Is your vision impaired or does applying makeup feel difficult? Or do you have under-eye bags that make you look tired or older? If so, it may be time to consider blepharoplasty. This surgical procedure is designed to improve the appearance of the upper or lower eyelids by removing excess skin, fat, or muscle. Here is everything you need to know about blepharoplasty in Belgium.

What is blepharoplasty?

Blepharoplasty is a common surgical procedure that rejuvenates the eyes and upper face. It corrects sagging eyelids and smooths under-eye bags or hollows. The procedure may target the upper eyelids, the lower eyelids, or both.

For the upper eyelids, the surgeon removes excess skin and any pockets of fat, recreating a natural crease in the eyelid. For the lower eyelids, the goal is to remove the fatty deposits that form puffiness or under-eye bags. In some cases, the surgeon also removes excess skin and tightens the lower eyelid.

These two procedures may be performed separately or together. Blepharoplasty can be carried out for cosmetic reasons or for functional purposes when excess skin obstructs the visual field. In the latter case, partial reimbursement may be provided by INAMI if an ophthalmologist confirms the medical necessity.

Who can undergo blepharoplasty?

Cosmetic blepharoplasty can be performed from the age of 18 if you feel self-conscious about the appearance of your eyelids. Common reasons include:

  • excess skin on the upper eyelids
  • fat deposits on the upper eyelids
  • loose or wrinkled skin on the lower eyelids
  • under-eye bags or puffiness on the lower eyelids

These signs can make the face appear tired or older. With age, eyelids naturally sag, which may worsen the concern or even cause discomfort.

How does blepharoplasty work in Belgium?

Before the procedure, you will attend an initial consultation with your surgeon to discuss the treatment. The surgeon will examine your eyelids and explain what can be done and what kind of results you can expect. This is also the moment to ask any questions and confirm that you are comfortable moving forward.

If you choose to proceed, your surgeon will explain the next steps, such as scheduling an ophthalmologist appointment, meeting with an anesthesiologist, and setting a surgery date. Preoperative photographs will also be taken.

On the day of the procedure, you may undergo local or general anesthesia depending on your preference. In most cases, local anesthesia with sedation is sufficient. The operation usually lasts between one and two hours.

The procedure will vary based on the type of blepharoplasty being performed:

  • For upper eyelid surgery, the surgeon makes an incision in the natural crease of the eyelid to ensure the scar is invisible. Excess skin, fat, and muscle are removed, and the incision is closed with sutures.
  • For lower eyelid surgery with excess skin, the incision is made just below the lashes, in the eyelid fold, and becomes invisible after healing. The surgeon removes the excess fat and skin and closes the incision.
  • For lower eyelid surgery without skin excess, the surgeon makes an incision inside the eyelid (transconjunctival). No external scar is visible, as the incision is inside the mucous membrane.

After surgery, the medical team will monitor you in the recovery room while the anesthesia wears off. Once stable, you will be able to return home a few hours later.

What to expect after surgery

Blepharoplasty is not a painful procedure, but you may experience mild discomfort for several days. A break from work and sports is recommended for about one week. Bruising and swelling typically appear around the eyelids the day after surgery and can take up to three months to fully subside.

You will have stitches on the treated areas, depending on the type of surgery performed. These are removed during a follow-up visit one week after the procedure. Afterward, gentle massage with a specialized cream, recommended by your doctor, may help the healing process.

A final follow-up visit is scheduled three months post-surgery to evaluate the final result and ensure the outcome meets your expectations.

How much does blepharoplasty cost in Belgium?

The cost of blepharoplasty in Belgium depends on the type of procedure (upper or lower eyelids) and the complexity of your case.

  • For two eyelids, prices typically range from €1,500 to €3,000
  • For all four eyelids, the cost ranges from €3,000 to €5,000

If you are undergoing eyelid surgery due to a functional problem, INAMI may provide reimbursement. You will need to consult an ophthalmologist to confirm visual impairment and validate the need for corrective surgery.

Blepharoplasty is a relatively straightforward procedure with minimal risks. It offers a refreshed, more youthful appearance and can even improve vision by expanding the visual field.
