What is an intragastric balloon and how does it work?

The intragastric balloon is a non-surgical medical device designed to help reduce excess weight temporarily. It is inserted into the stomach during a short procedure and then filled with a saline solution, occupying about one-third of the stomach’s volume. This reduces hunger, promotes a feeling of fullness, and facilitates weight loss. When combined with healthy eating and regular exercise, patients can typically lose between 15 and 20 kilograms over six months.

Who is a good candidate for the intragastric balloon?

This treatment is intended for individuals with a body mass index (BMI) between 27 and 35, typically classified as non-morbid obesity, who have struggled to lose weight despite multiple efforts. It’s particularly helpful for those who tend to eat large portions rather than those who snack frequently.

In some cases, patients with a BMI over 40 may also be eligible if weight loss is required before undergoing another surgical procedure.

However, to be considered for the balloon, you must:

  • Be in good overall health
  • Not suffer from severe heart, liver, lung, or kidney disease
  • Not have severe reflux, blood clotting disorders, or serious mental health conditions

Types of intragastric balloons and how they are placed

There are several types of intragastric balloons available, with differences in how they are inserted and removed:

Swallowable balloon

This capsule-shaped balloon is swallowed with water, requiring no anesthesia or endoscopy. Once in the stomach, it is filled with saline through a fine catheter, which is then removed. The balloon naturally deflates and is expelled via the digestive system after several months.

Endoscopically placed balloon

These types require insertion and removal via endoscopy, typically under light anesthesia. The balloon is placed in the stomach through the mouth and then filled with saline. After 6 to 12 months, it is removed using the same method.

Procedure steps

For the swallowable balloon:

  • No anesthesia is required
  • The process takes around 15 to 30 minutes
  • Patients must fast for 6 hours for solids and 3 hours for liquids
  • Once the capsule reaches the stomach, an X-ray confirms placement
  • The balloon is filled with saline and the catheter is gently removed

For endoscopic balloons:

  • Performed under local or general anesthesia
  • The stomach is assessed via initial endoscopy
  • The balloon is inserted, positioned, and filled with saline
  • A follow-up program is recommended throughout the 6 to 12 months the balloon remains in place

Is it effective for long-term weight loss?

The intragastric balloon is highly effective for short-term weight loss. Most patients lose between 10 and 30% of their initial weight in the first six months. However, long-term success depends on maintaining a healthy diet and regular physical activity after the balloon is removed.

The benefits go beyond aesthetics. Weight loss often boosts self-confidence and reduces the risk of conditions linked to obesity, such as type 2 diabetes and cardiovascular disease.

Are there any risks or side effects?

As with any medical procedure, the intragastric balloon involves some risks. The most common side effects include:

  • Nausea and vomiting
  • Abdominal discomfort
  • Gastric irritation or mild bleeding

More rarely, complications may include:

  • Gastric ulcers or perforation
  • Balloon migration leading to intestinal obstruction

These complications are uncommon and typically manageable with prompt medical care.
